Facility Use Procedures
Please Follow the Facility Use Procedures Below:
- External users/groups shall create an account by visiting https://az11.mlschedules.com/Register.aspx . The process includes and requires a valid Certificate of Liability Insurance (COI) with Tempe Union High School District identified as the additional named insured, proof of non-profit status (if required by Class), and a Photo ID. Accounts cannot be created without proper documentation.
- External users/groups with an account, please complete the External Facility Request Process by visiting https://az11.mlschedules.com/Login.aspx . Requests will not be granted without proper documentation.
3. If no response is received within three (3) business days, follow up via email with the Facility Use Coordinator at FacilityRentals@tempeunion.org.
4. If the facility request is approved by the requested site or school administration:
a. A Facility Use Quote may be sent to the client.
b. A Facility Use Agreement / Terms of Conditions containing our policies, regulations, and procedures will be sent to the client for initials and signature.
c. If the facility request is not approved, the Facility Use Coordinator will notify the client.
d. The invoice will be sent along with the signed permit. Payment, in full, for rentals is due at least 72 hours prior to event or first date of events. Failure to fulfill the invoice will result in cancellation of the event, unless prior payment arrangements have been made with the District. Payments will be accepted via credit/debit card, or check. Please make checks payable to Tempe Union High School District Civic Center Fund.
e. Should the event details change, the Site Supervisor will communicate with the Facility Use Coordinator and an additional invoice may be sent for event incidentals. (extended event time, additional services provided, excessive trash, etc.)
