EVIT PERSONAL TRANSPORTATION FORM - PDF (for junior students onlySchool administration will continue our policy of allowing senior students to leave campus during lunchtime, along with those juniors who qualified for the Aztec incentive program and who selected off-campus lunch, with parental permission. The release form below must be downloaded, completed, signed by parents, and notarized in order for the student to leave campus during the lunch hours. This form must be turned in during book distribution in order for a student to receive the correct ID that will allow them to check out with campus security.
