Child Find & Evaluation

Tempe Union High School District is required by federal and state law (34 CFR 300.220.34 CFR 300.121) to conduct a “Child Find” effort as a means to locate, identify, and evaluate all resident children with disabilities, including children with disabilities attending home schools or private schools, regardless of the severity of their disabilities. The purpose of Child Find is to locate these children so that a free appropriate public education can be made available. Upon request, Tempe Union High School District will screen any resident child in grades 9 through 12 to determine whether a special education referral is appropriate.  If a referral for an evaluation is appropriate, students are evaluated and eligible students are entitled to a free and appropriate education, including specially designed instruction for students with disabilities.  For more information about Child Find, see AZED.gov/SpecialEducation/Parents/AZ-Find


CHILD FIND & EVALUATION PROCEDURES

Students Enrolled in the Tempe Union High School District
If your child is enrolled at a Tempe Union High School and you suspect that your child has a disability requiring special education and related services and your child does not currently have an IEP, contact the school psychologist at your child’s school.  The school psychologist and a team, including the parent and student, will discuss the concerns and determine whether an evaluation is appropriate and the next step in the evaluation process, if applicable.

Students Attending Private or Home Schools
If you live within the boundaries of Tempe Union High School District or your child attends a private school within the Tempe Union High School District boundaries, you may request a screening for students in grades 9-12 by completing this form.

For Children Ages 0-5
For Children - Birth to Age 3
Submit a referral to the AZ Early Intervention Program (AZEIP) online by using this link: https://azeip.azdes.gov/AzEIP/AzeipRef/Forms/Categories.aspx

For Children - 3 to Age 5 (not yet enrolled in kindergarten)
Contact your home elementary school district.

For Children in Kindergarten through 8th grade
Contact the school psychologist at your child’s school or the elementary school district in which you reside.
